Welcome to your new career and your union! Before you get started, there are a few pieces of paperwork to complete to make everything official. All forms are completed and signed electronically using DocuSign, a secure electronic signature platform.
AFA Membership Application
This form is your official application for AFA membership and is necessary to ensure that you have all of the rights of a union member.
Who needs to complete this form? All new hire Flight Attendants need to complete the AFA Membership Application.
AFA Dues Check Off Form
This form will set you up for automatic payroll deduction of your monthly AFA dues. Dues will be deducted once per month started after your fourth full calendar month of employment.
Who needs to complete this form? All new hire Flight Attendants need to complete the AFA Dues Check Off Form.
Alaska Union Dues Stop Payment Form
This form is for anyone who is coming from another union-represented position within Alaska Air Group (AAG). This includes Alaska CSAs, Reservations Agents, RSAs; Horizon Flight Attendants; and McGee Air Service Ramp Agents. This form will be submitted to Alaska Airlines payroll on your behalf and will stop payroll deductions for union dues from your previous union.
Who needs to complete this form? Only complete the union dues stop payment form if you were employed in another union-represented position at AAG just prior to starting Initial Training.
